Job Description
Join a dynamic and growing home health care agency in Roseburg on a contract assignment. Local and travel candidates will be considered. We are currently seeking skilled and compassionate Registered Nurses (RNs) to fill multiple contract positions. OASIS proficiency is REQUIRED.
Key Responsibilities:
- Deliver high-quality nursing care in the home setting, following established care plans.
- Conduct initial patient assessments and develop customized care plans in coordination with healthcare teams.
- Administer medications, provide wound care, and manage IV therapy as needed.
- Educate patients and families on disease processes, treatment options, and home care best practices.
- Maintain accurate and timely documentation of patient care in the electronic medical records (EMR) system.
- Monitor patient progress and adjust care plans as necessary to achieve optimal outcomes.
- Communicate effectively with physicians, therapists, and other care providers to ensure comprehensive care.
- Adhere to all safety protocols and guidelines in delivering home health services.
Qualifications:
- Current and unrestricted RN license
- Previous geriatric home health experience
- Strong OASIS proficiency
- Experience with HCHB a big plus
- Strong clinical assessment and care planning skills.
- Familiarity with EMR systems and documentation standards.
- Ability to work autonomously and adapt to changing patient needs.
- Excellent organizational and time management abilities.
- Reliable transportation and a valid driver’s license.
