Job Description

Join a dynamic and growing home health care agency in Roseburg on a contract assignment. Local and travel candidates will be considered. We are currently seeking skilled and compassionate Registered Nurses (RNs) to fill multiple contract positions. OASIS proficiency is REQUIRED.

Key Responsibilities:

  • Deliver high-quality nursing care in the home setting, following established care plans.
  • Conduct initial patient assessments and develop customized care plans in coordination with healthcare teams.
  • Administer medications, provide wound care, and manage IV therapy as needed.
  • Educate patients and families on disease processes, treatment options, and home care best practices.
  • Maintain accurate and timely documentation of patient care in the electronic medical records (EMR) system.
  • Monitor patient progress and adjust care plans as necessary to achieve optimal outcomes.
  • Communicate effectively with physicians, therapists, and other care providers to ensure comprehensive care.
  • Adhere to all safety protocols and guidelines in delivering home health services.

Qualifications:

  • Current and unrestricted RN license
  • Previous geriatric home health experience
  • Strong OASIS proficiency
  • Experience with HCHB a big plus
  • Strong clinical assessment and care planning skills.
  • Familiarity with EMR systems and documentation standards.
  • Ability to work autonomously and adapt to changing patient needs.
  • Excellent organizational and time management abilities.
  • Reliable transportation and a valid driver’s license.
