Job Description:
Sunbelt Staffing is hiring a dedicated Home Health Licensed Practical Nurse (LPN) for an assignment in Richmond, KY. In this role, you’ll provide compassionate, high-quality care to patients in their homes, manage a caseload effectively, and ensure accurate documentation using Homecare Homebase. This position is open to both local candidates and travelers seeking an opportunity in a vibrant and supportive community.
Key Responsibilities:
- Deliver skilled nursing care to home health patients based on individualized care plans.
- Administer medications, perform wound care, and provide patient education.
- Monitor patient progress, updating care plans as necessary.
- Complete accurate documentation in Homecare Homebase.
- Collaborate with healthcare professionals to deliver comprehensive patient care.
Qualifications:
- Active LPN license in Kentucky or compact state licensure.
- At least 3 years of LPN experience, with 1 year of recent home health care experience.
- Proficiency with Homecare Homebase required.
- Current BLS certification.
- Reliable transportation and valid driver’s license.
- Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission.
