Shift: Monday-Friday, with potential on-call

Job Description:
Sunbelt Staffing is seeking an experienced Home Health Registered Nurse (RN) for a 13-week contract assignment in Portsmouth, NH. This role requires proficiency in OASIS documentation and experience with Homecare Homebase software, providing an excellent opportunity to deliver top-quality care to patients in the comfort of their homes.

Key Responsibilities:

  • Perform detailed patient assessments and document using OASIS standards
  • Develop and implement individualized care plans to address patient needs
  • Provide nursing care, including wound management and medication administration
  • Collaborate with interdisciplinary teams to ensure effective patient outcomes
  • Educate patients and caregivers on self-care and disease management

Qualifications:

  • Active RN license in New Hampshire
  • Proficiency in OASIS documentation and experience with Homecare Homebase required
  • 2 years of Home Health experience preferred
  • Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
  • Locals and travelers both accepted
