Job Description:
Sunbelt Staffing is hiring a dedicated Home Health Registered Nurse (RN) for a 13-week contract in Londonderry, NH. This position requires proficiency in OASIS documentation and experience with Homecare Homebase software, offering a chance to provide exceptional patient care in a supportive home health environment.

Key Responsibilities:

Conduct detailed patient assessments and document findings using OASIS
Develop and execute personalized care plans for patients
Deliver direct nursing care, including medication administration and wound care
Work collaboratively with the healthcare team to ensure high-quality outcomes
Educate patients and their families on self-care and wellness strategies

Qualifications:

Active RN license in New Hampshire
OASIS proficiency and experience with Homecare Homebase required
2 years of Home Health experience preferred
Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
Locals and travelers both accepted
