Job Description:
Sunbelt Staffing is seeking a Home Health Registered Nurse (RN) for a 13-week contract assignment in Indianapolis, IN. This position is perfect for an RN experienced in OASIS documentation and proficient with Homecare Homebase software. Deliver patient-focused care while working in a supportive and dynamic home health environment.

Key Responsibilities:

Conduct comprehensive patient assessments and document findings using OASIS standards
Develop and manage individualized care plans to address patient needs
Provide nursing care, including wound care, medication administration, and health monitoring
Collaborate with an interdisciplinary team to achieve optimal patient outcomes
Educate patients and caregivers on self-care techniques and health management strategies

Qualifications:

Active RN license in Indiana
Proficiency in OASIS documentation and experience with Homecare Homebase required
2 years of Home Health experience preferred
Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
Locals and travelers both accepted
