Job Description:
Sunbelt Staffing is seeking an experienced Home Health Registered Nurse (RN) for a 13-week contract in Hampton, VA. This role requires proficiency in OASIS documentation and experience with Homecare Homebase software. Join a dynamic healthcare team dedicated to providing quality patient care in the home health setting.

Key Responsibilities:

Perform comprehensive patient assessments and document findings using OASIS standards
Develop and implement personalized care plans to address individual needs
Provide direct nursing care, including wound care and medication administration
Collaborate with a multidisciplinary team to ensure excellent patient outcomes
Educate patients and their families on self-care and disease management

Qualifications:

Active RN license in Virginia
Proficiency in OASIS documentation and experience with Homecare Homebase required
2 years of Home Health experience preferred
Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
Locals and travelers both accepted
