Full-Time Physical Therapist (PT) Contract in American Fork, UT| Geriatric Rehab

Join a top-tier rehab team as a Physical Therapist (PT) in American Fork, UT! We are hiring a PT for a 12-week contract focused on helping older adults in post-acute care regain mobility and independence. If you have a background in SNF or geriatric therapy, this is your chance to make a real impact.

Position Highlights:

  • Start: ASAP

  • Schedule: MondayFriday, 8-hour days (40 hrs/week)

  • Setting: One primary facility, occasional nearby outpatient visits

  • Patient Base: Seniors (60+), post-hospital recovery

Your Responsibilities:

  • Evaluate physical capabilities and design rehab programs

  • Provide direct therapy services to improve function and safety

  • Collaborate with interdisciplinary teams

  • Maintain detailed documentation for compliance and care continuity

Requirements:

  • Utah PT license in good standing

  • Experience working in SNFs, rehabilitation hospitals, or post-acute settings

  • Strong interpersonal skills and a patient-centered mindset
