Job Description

Sunbelt Staffing is seeking a compassionate and experienced Home Health Case Manager for a 13-week travel assignment in West, Allis. This opportunity combines rewarding patient care with the chance to explore the rich blend of history, culture, and community of West Allis.

Job Description:

As a Home Health Case Manager, you will provide comprehensive care to patients, offering comfort, support, and guidance. Your responsibilities include:

Developing and implementing individualized care plans
Managing patient symptoms and medication
Coordinating care with interdisciplinary teams

Qualifications:

Active RN license in WI or compact state
Minimum of1 year of RN experience
BLS required
Strong clinical skills including IV therapy, wound care, pain management, and end-of-life care
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
