Job Description:
Sunbelt Staffing is seeking a dedicated Home Health Registered Nurse (RN) for a 13-week contract in Brooklyn Park, MN. This role requires proficiency in OASIS documentation and experience with Homecare Homebase software, providing an excellent opportunity to deliver quality patient care while working in a dynamic and supportive environment.

Key Responsibilities:

Conduct thorough patient assessments and document using OASIS
Develop individualized care plans tailored to each patient’s needs
Provide hands-on nursing care, including medication administration and wound care
Collaborate with the healthcare team to achieve the best patient outcomes
Educate patients and families on health maintenance and self-care strategies

Qualifications:

Active RN license in Minnesota
Proficiency in OASIS and experience with Homecare Homebase required
2 years of Home Health experience preferred
Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
Locals and travelers both accepted
