Sunbelt Staffing, a nationwide healthcare staffing agency, is seeking a compassionate and experiencedHome Health Registered Nurse (RN) for a 13-week contract position inWoodstock, GA. The successful candidate will join our client, a leading nationwide home health agency, and provide quality patient care in a home-based setting.

Contract Details:

  • Location: Woodstock, GA
  • Company: Sunbelt Staffing
  • Contract Length: 13 Weeks (with possible extension upon successful start and completion)
  • Hours: Full-time, 40 hours per week, Monday-Friday, 8am-5pm, with occasional weekends and on-call duty (2-3 times per month)
  • Productivity: 30per week
  • Compensation: Negotiable, based on experience and territory budget
  • Mileage Reimbursement: .48c from patient home to home, up to 150 miles per week expected travel
  • Blended Pay Packages: Available for travelers
  • Benefits: Medical, dental, vision, 401k match after 90 days

Job Responsibilities

  • Conduct comprehensive patient assessments using OASIS, ensuring accurate documentation and compliance with agency standards.
  • Develop and implement individualized care plans, coordinating with physicians, physical therapists, and other healthcare professionals.
  • Administer medications, monitor vital signs, and provide skilled nursing care, including wound care, IV therapy, and catheter care.
  • Educate patients and their families about care plans, medication management, and other health-related topics.
  • Maintain accurate patient records in Homecare Homebase (HCHB), ensuring compliance with regulatory requirements.
  • Travel to patient homes within the territory, using reliable transportation.
  • Participate in interdisciplinary team meetings and case conferences to ensure coordinated and effective patient care.
  • Adhere to Joint Commission standards and other regulatory requirements.

Requirements

  • Active Registered Nurse (RN) license in the state of Georgia.
  • Minimum of 1 year of home health experience.
  • Proficiency in OASIS and Homecare Homebase (HCHB).
  • Current American Heart Association or American Red Cross Basic Life Support (BLS) certification.
  • Reliable transportation with valid driver's license.
  • Ability to meet Joint Commission onboarding standards upon offer.
