Sunbelt Staffing is seeking a full-time contract Nurse Practitioner (NP) to provide exceptional care for insurance members in skilled nursing and assisted living facilities in the Corvallis, OR area.

Key Responsibilities:

  • Provide comprehensive care for patients, focusing on identifying gaps in care and assisting in managing their overall care plans.
  • Perform medication reconciliation, medical history reviews, ADLs evaluations, nutritional and cognitive assessments, and more.
  • Visit patients who have recently transitioned to facilities as part of their hospital discharge plans to ensure smooth and effective care continuity.

What You Bring:

  • Active Oregon NP License and DEA certification.
  • ANCC or AANP National Certification in Family, Adult, Geriatric, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care.
  • At least 1 year of Nurse Practitioner experience in a clinical setting, with a passion for caring for elderly and adult populations.
  • Willingness to travel within the Corvallis, OR area to various skilled nursing facilities.
