Position: Nurse Practitioner
Location: Medford, OR
Program: Institutional Special Needs Plan (ISNP)
Setting: Nursing Home / Assisted Living Facilities

Sunbelt Staffing is looking for a Nurse Practitioner to join an ISNP program in Medford, OR, providing care for patients in nursing homes and assisted living facilities. This role focuses on improving residents' quality of life through personalized care and collaboration with facility staff.

Key Responsibilities:

  • Perform comprehensive clinical assessments on residents.
  • Develop individualized care plans and medical treatment strategies.
  • Provide on-site primary, preventative, and urgent care.
  • Stratify high-risk patients for more frequent visits and enhanced care.
  • Collaborate with primary care physicians and facility staff.
  • Communicate with residents’ families and responsible parties after visits.
  • Assist with smooth transitions for patients entering the facility post-hospital discharge.
  • Manage a panel of patients across multiple nursing facilities.

Qualifications:

  • Active Nurse Practitioner license in Oregon.
  • Experience in geriatric or long-term care is a plus.
  • Excellent communication and collaboration skills.
