Position: Home Health Registered Nurse
Schedule: Full-Time

Job Overview:
We are seeking a skilled and dedicated Home Health RN with expertise in OASIS and Home Care Home Base. Join our dynamic team to deliver high-quality care and provide patient education in the comfort of their homes.

Responsibilities:

Perform comprehensive patient assessments using OASIS documentation.
Develop individualized care plans and coordinate care with multidisciplinary teams.
Administer medications, IV therapy, and wound care.
Educate patients and families on managing chronic illnesses and post-acute recovery.
Maintain accurate documentation in Home Care Home Base.

Qualifications:

Current Georgia RN license.
Minimum of 1 year of recent home health experience.
Proficiency in OASIS and Home Care Home Base required.
Strong organizational and communication skills.
