Home Health LPN – 13-Week Contract in Durham, NC
Contract Length: 13 Weeks
Shift Hours: Full-time, Monday - Friday

Position Overview:
We are seeking a dedicated and compassionate Licensed Practical Nurse for a home health contract in Durham, NC. The ideal candidate will bring expertise in patient-centered care within a home setting and have experience using HomeCare HomeBase (HCHB) for documentation. OASIS experience is not required for this position.

Responsibilities:

Provide high-quality nursing care to patients in their homes.
Document patient information and care plans accurately using HCHB software.
Administer medications and treatments as prescribed.
Monitor patient conditions and report changes to the care team.
Educate patients and families on health management and care routines.
Work collaboratively with the healthcare team to ensure optimal patient outcomes.

Requirements:

Active LPN License in North Carolina or compact state.
Minimum of 1 year of recent home health experience.
Proficiency in HomeCare HomeBase (HCHB) documentation required.
Strong communication and organizational skills.
BLS certification required.
