Home Health LPN – 13-Week Contract in Lee’s Summit, MO
Contract Length: 13 Weeks
Shift Hours: Full-time, Monday - Friday

Position Overview:
We are looking for a compassionate and dedicated LPN for a home health contract in Lee's Summit, MO. The ideal candidate will have experience with HomeCare HomeBase (HCHB) software, but OASIS experience is not required. The position involves providing in-home care to patients, ensuring they receive high-quality treatment while maintaining a positive and empathetic environment.

Responsibilities:

Provide high-quality, patient-centered care to individuals in their homes.
Administer medications, dress wounds, and assist with other daily activities as required.
Monitor patient vital signs and report changes in condition to the care team.
Use HomeCare HomeBase (HCHB) for patient documentation and charting.
Coordinate care with physicians, family members, and other healthcare professionals.

Requirements:

Active LPN License in Missouri or compact state.
Minimum of 1 year of recent home health experience preferred.
Experience using HomeCare HomeBase (HCHB) software required.
Must be able to work independently and manage patient care effectively.
Excellent communication and interpersonal skills.
BLS certification required.
