Position Type: Full-Time

Position Overview:
We are seeking an experienced Home Health Registered Nurse (RN) to join our dedicated team in Hillsboro, MS. The ideal candidate will have a strong background in home health care, with expertise in OASIS documentation and proficiency in Homecare Home Base software.

Key Responsibilities:

Provide in-home patient care and clinical assessments.
Complete and submit OASIS documentation accurately and promptly.
Utilize Homecare Home Base software to document and manage patient care plans.
Educate patients and families on health conditions and care strategies.
Coordinate care with the interdisciplinary team to ensure optimal outcomes.

Qualifications:

Active RN license in the state of Missouri.
Minimum of 1 year of home health nursing experience.
Proficiency with OASIS documentation and Homecare Home Base software (required).
Current BLS certification.
Strong interpersonal and communication skills.
