Job Title: Home Health Registered Nurse (RN)
Location: Medford, NY

Shift: Monday-Friday, with potential on-call

Job Description:
Sunbelt Staffing is looking for a dedicated Home Health Registered Nurse (RN) for a 13-week contract in Medford, NY. In this role, you’ll use your expertise in OASIS documentation and Homecare Homebase software to provide exceptional care to patients in the comfort of their own homes.

Key Responsibilities:

Perform in-depth patient assessments and document care using OASIS standards
Develop customized care plans to meet individual patient needs
Deliver skilled nursing services, including wound care, medication management, and health monitoring
Collaborate with the healthcare team to ensure optimal patient outcomes
Provide patient and family education on disease management and health promotion

Qualifications:

Active RN license in New York
Proficiency in OASIS documentation and experience with Homecare Homebase required
Minimum of 2 years of Home Health nursing experience preferred
Ability to pass background check, drug screening, and onboarding in compliance with Joint Commission standards
