Location: Staunton, Virginia
Position: Home Health Registered Nurse
Schedule: Full-Time

Role Summary:
We are seeking an experienced Home Health RN for our team in Staunton, VA. Applicants must be proficient in OASIS and Home Care Home Base to provide exceptional care and support in a home health setting.

Responsibilities:

Perform OASIS-based patient assessments and create individualized care plans.
Provide skilled nursing care, including IV therapy, wound care, and medication management.
Educate patients and families to ensure optimal recovery and disease management.
Document all care activities using Home Care Home Base.
Collaborate with multidisciplinary teams to achieve positive patient outcomes.

Qualifications:

Active Virginia RN license.
Proficiency in OASIS and Home Care Home Base required.
Minimum 1 year of recent home health experience.
Strong organizational and communication skills.
