Location: Wentworth, New Hampshire
Position: Licensed Practical Nurse (Home Health)
Schedule: Full-Time

Job Summary:
Join our dynamic home health care team in Wentworth, NH, as a Home Health LPN. This position requires prior home health experience and proficiency with Home Care Home Base for documentation. Apply quickly, as these positions fill fast!

Key Responsibilities:

Provide compassionate, in-home nursing care for a diverse patient population.
Perform wound care, medication administration, and IV therapy as needed.
Complete patient assessments and document care accurately using Home Care Home Base.
Educate patients and caregivers about managing chronic illnesses and recovery.
Collaborate with RNs and other healthcare providers to ensure seamless care delivery.

Qualifications:

Current LPN license in New Hampshire.
Minimum 1 year of recent home health experience required.
Proficiency in Home Care Home Base is mandatory.
Strong communication and organizational skills.
