Job Title: Home Health Registered Nurse (RN)
Location: Shelton, CT

Shift: Monday-Friday, with possible on-call

Job Description:
Sunbelt Staffing is hiring an experienced Home Health RN for a travel assignment in Shelton, CT. This role requires expertise in OASIS documentation and proficiency in Homecare Homebase. If you are a dedicated nurse seeking a rewarding role, apply today!

Key Responsibilities:

  • Conduct in-home patient assessments and develop individualized care plans
  • Provide skilled nursing care, including wound care, IV therapy, and medication management
  • Ensure accurate and timely OASIS documentation using Homecare Homebase
  • Educate patients and families on home health best practices
  • Participate in on-call rotations as needed

Qualifications:

  • Active RN license in Connecticut
  • Minimum of 2 years of recent home health experience
  • Strong proficiency in OASIS and Homecare Homebase required
  • Current BLS certification (AHA)
  • Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
