Schedule: Full-Time | Flexible Schedule Required

We are seeking a compassionate and experienced Registered Nurse (RN) to join our dynamic home health team in Phoenix, AZ. In this role, you will deliver skilled nursing care to clients of all ages in their homes and work collaboratively with an interdisciplinary team to provide personalized and high-quality care. Responsibilities:

  • Assess patients to identify physical, psychosocial, and environmental needs.

  • Develop, implement, and evaluate individualized Plans of Care (POC).

  • Ensure continuity and quality of care through accurate, timely documentation.

  • Provide patient and family education on health, medications, and home safety.

  • Supervise and guide HHAs, LVNs, and other support staff.

  • Assess pain levels and implement appropriate pain management strategies.

  • Arrange referrals and coordinate with third-party payers as needed.

  • Participate in on-call rotation and provide on-call services as assigned.

  • Communicate effectively with patients, families, providers, and the care team.

  • Attend interdisciplinary team meetings and contribute to care planning.

Qualifications:

  • Current Registered Nurse (RN) license Required

  • CPR Certification Required

  • Valid driver's license and reliable transportation with insurance Required

  • 12 years RN experience in a clinical care setting Required

  • 2+ years of home health experience Required

  • Experience with Homecare Homebase Software and OASIS Required

  • CHPN certification Preferred

  • Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program Required

  • Bachelor of Science in Nursing (BSN) Preferred

Skills & Abilities:

  • Strong clinical knowledge, including medication administration and wound care

  • Proficiency with venipuncture, IV therapy, and durable medical equipment

  • Ability to assess, document, and respond to patient needs in home settings

  • Organized, adaptable, and able to prioritize under pressure

  • Exceptional communication and teamwork skills

  • Committed to professional growth and maintaining confidentiality
