Shift: Monday-Friday, with possible on-call

Job Description:
Sunbelt Staffing is seeking an experienced Home Health RN to join a top-tier healthcare team in Opelousas, LA. This role requires strong clinical skills in home health, proficiency with OASIS documentation, and experience using Homecare Homebase. Key Responsibilities:

Conduct in-home patient assessments and create personalized care plans
Provide skilled nursing care, including medication management, wound care, and IV therapy
Ensure timely and accurate OASIS documentation using Homecare Homebase
Educate patients and caregivers on disease management and treatment plans
Participate in on-call rotations as needed

Qualifications:

Active RN license in Louisiana
Minimum of 2 years of recent home health experience
Proficiency in OASIS and Homecare Homebase required
Current BLS certification (AHA)
Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
