Job Title: Home Health Registered Nurse (RN)
Location: Lynchburg, VA

Type: Travel/Contract

Duration: 13 Weeks

Shift: Monday-Friday, with possible on-call

Job Description:
Sunbelt Staffing is hiring a Home Health RN for a travel contract in Lynchburg, VA. This role requires expertise in OASIS documentation and experience with Homecare Homebase. If you have a passion for patient-centered care and thrive in an independent, flexible home health setting, apply today!

Key Responsibilities:

  • Conduct comprehensive in-home assessments and create personalized care plans
  • Provide skilled nursing care, including post-surgical care and chronic disease management
  • Maintain accurate OASIS documentation using Homecare Homebase
  • Educate patients and families on health management and medication adherence
  • Participate in on-call duties as required

Qualifications:

  • Active RN license in Virginia
  • Minimum of 2 years of recent home health experience
  • Strong proficiency in OASIS and Homecare Homebase required
  • Current BLS certification (AHA)
  • Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
