Sunbelt Staffing, in collaboration with a prominent Nationwide Home Health Agency, is actively seeking a committed Registered Nurse (RN)to join our clients team on a contractual basis in Granite City, IL.

This 13-week contract presents the possibility of extension contingent upon exemplary performance, offering you stability and avenues for professional advancement within the dynamic healthcare landscape.

Job Details:

• Location: Granite City, IL
• Company: Sunbelt Staffing, on behalf of a Nationwide Home Health Agency
• Position Type: Contract, 13-week term with possibility of extension upon successful completion
• Schedule: Monday-Friday, 8:00 AM - 5:00 PM with occasional weekends and on-call
• Compensation: Competitive salary, mileage reimbursement, medical, dental, vision, and 401k benefits after 90 days
• Productivity: 25 points per week
• Mileage: .48c visit to visit, 150-250 miles per week expected travel

Job Responsibilities:

• Conduct home health visits to patients within territory,and its surrounding areas.
• Perform comprehensive assessments of patients' health status, including physical, emotional, and environmental factors influencing their well-being.
• Develop and execute personalized care plans tailored to meet the unique needs and goals of each patient, in collaboration with the healthcare team and under physician guidance.
• Administer skilled nursing care, encompassing wound management, medication administration, IV therapy, and patient education on disease management and preventative care measures.
• Maintain accurate and timely documentation of all patient interactions, assessments, interventions, and outcomes utilizing Oasis and Homecare Homebase software platforms.
• Foster interdisciplinary collaboration by actively participating in team meetings, sharing insights, and coordinating care transitions to ensure continuity and effectiveness of care delivery.
• Establish and maintain open lines of communication with patients, families, and caregivers, providing support, education, and advocacy to enhance patient understanding and engagement in their healthcare journey.

Job Requirements:

• Active IllinoisRegistered Nurse (RN) License.
• Active American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) certification.
• Minimum of 1 year of experience in home health nursing in a non private duty setting.
• Minimum of 1 year of experience with Oasis and Homecare Homebase software, proficiency required.
• Excellent clinical assessment and critical thinking skills.
• Strong communication and interpersonal abilities.
• Reliable transportation and willingness to travel within assigned territory.

Please Note: Due to this being a contract opportunity with limited training, proficiency in Oasis and Homecare Homebase are required.
