Shift: Monday–Friday, with possible on-call

Job Description:
Sunbelt Staffing is hiring a dedicated Home Health Registered Nurse (RN) for a contract assignment in Bridgewater, MA. This role involves providing skilled nursing care to patients in their homes, managing care plans, and ensuring accurate OASIS documentation. The ideal candidate will be proficient with Homecare Homebase and have strong clinical judgment to deliver quality care independently.

Key Responsibilities:

Perform in-home patient assessments and develop care plans
Deliver direct nursing care, including wound care, IV therapy, and medication management
Complete and submit accurate OASIS documentation
Utilize Homecare Homebase for efficient electronic charting
Coordinate with healthcare providers and family members to support patient care
Participate in on-call rotations as needed

Qualifications:

Active Massachusetts RN license
Minimum of 2 years of home health nursing experience
Proficiency with OASIS and Homecare Homebase
BLS certification (AHA)
Reliable transportation, valid driver’s license, and car insurance
Ability to pass a background check, drug screen, and onboarding in compliance with the Joint Commission
