Location: Salisbury, MD

Duration: 13 weeks

Shift: Monday-Friday, with potential on-call

Job Description:
Sunbelt Staffing is hiring a skilled Home Health Licensed Practical Nurse (LPN) for an assignment in Salisbury, MD. This role involves delivering compassionate care to patients in their homes, managing a diverse caseload, and documenting all care accurately using Homecare Homebase. Key Responsibilities:

  • Provide personalized, skilled nursing care based on patient care plans.
  • Administer medications, perform wound care, and educate patients and families on health management.
  • Monitor and report changes in patient conditions, updating care plans as necessary.
  • Ensure accurate, timely documentation in Homecare Homebase.
  • Work collaboratively with the healthcare team to deliver comprehensive patient care.

Qualifications:

  • Active LPN license in Maryland or compact state licensure.
  • At least 3 years of LPN experience, with a minimum of 1 year of recent home health care experience.
  • Proficiency with Homecare Homebase is required.
  • Current BLS certification.
  • Reliable transportation and a valid drivers license.
  • Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission.
