Job Title: Home Health Licensed Practical Nurse (LPN)
Location: Salisbury, MD

Duration: 13 weeks

Shift: Monday-Friday, with potential on-call

Job Description:
Sunbelt Staffing is seeking an experienced Home Health Licensed Practical Nurse (LPN) for an assignment in Salisbury, MD. In this role, you will provide quality nursing care to patients in their homes, manage a diverse caseload, and ensure accurate documentation using Homecare Homebase. Both local candidates and travelers are welcome to apply for this rewarding opportunity.

Key Responsibilities:

Provide skilled nursing care following individualized home health care plans.
Administer medications, perform wound care, and educate patients and families.
Monitor patient conditions and adjust care plans as needed.
Maintain accurate and timely documentation in Homecare Homebase.
Collaborate with healthcare professionals to ensure comprehensive, patient-centered care.

Qualifications:

Active LPN license in Maryland or compact state licensure.
Minimum of 3 years of LPN experience, with at least 1 year of recent home health care experience.
Proficiency with Homecare Homebase is required.
Current BLS certification.
Reliable transportation and a valid drivers license.
Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission.
