Position Details:

  • Job Title: Home Health LPN
  • Location:Lexington, KY
  • Schedule: Full-time, 40 hours per week (Monday-Friday, 8:00 AM - 5:00 PM)
  • On-Call: One weekend per month
  • Patient Population: Adult and Geriatric
  • Productivity: 25-32 units per week
  • Mileage Reimbursement: $0.48 per mile (visit to visit), 150-250 miles per week expected
  • Benefits: Medical, Dental, Vision, 401(k) after 90 days
  • Pay: Negotiable based on territory budget and candidate experience

Key Responsibilities:

  • Patient Care: Conduct thorough assessments and provide skilled nursing care to patients in their homes, ensuring all care plans are effectively executed.
  • Homecare Homebase: Utilize Homecare Homebase (HCHB) software to document patient visits, manage care plans, and communicate with the healthcare team.
  • Coordination: Collaborate with physicians, physical therapists, and other healthcare professionals to ensure a cohesive approach to patient care.
  • On-Call Support: Provide weekend on-call support on a rotational basis, addressing urgent patient needs.
  • Patient Education: Educate patients and their families on disease management, medication administration, and lifestyle modifications to promote optimal health.

Requirements:

  • Active KentuckyLicensed Practical Nurse (LPN) License
  • Current Basic Life Support (BLS) certification from the American Heart Association or American Red Cross
  • Minimum of 1 year of experience as an LPN in a home health setting
  • Proficiency in Homecare Homebase (HCHB) software
  • Reliable transportation and a valid drivers license
  • Ability to complete Joint Commission onboarding standards
