Location: Lee's Summit, MO

Duration: 13 weeks

Shift: Monday-Friday, with potential on-call

Job Description:
Sunbelt Staffing is looking for a dedicated Home Health Licensed Practical Nurse (LPN) for an assignment in Lee's Summit, MO. In this role, you will provide exceptional nursing care to patients in their homes, manage a caseload, and ensure accurate documentation using Homecare Homebase. This position is open to both local candidates and travelers, offering a great opportunity to work in a growing and welcoming community.

Key Responsibilities:

  • Provide skilled nursing care to home health patients following individualized care plans.
  • Administer medications, perform wound care, and educate patients and families.
  • Monitor patient progress, updating care plans as needed.
  • Accurately document all patient interactions and care using Homecare Homebase.
  • Collaborate with the healthcare team to ensure high-quality, patient-centered care.

Qualifications:

  • Active LPN license in Missouri (MO) or compact state licensure.
  • At least 3 years of LPN experience, including 1 year of recent home health care experience.
  • Proficiency with Homecare Homebase required.
  • Current BLS certification.
  • Reliable transportation and a valid driver’s license.
  • Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission.
