Location: Bridgewater, MA

Duration: 13 weeks

Shift: Monday-Friday, with potential on-call

Job Description:
Sunbelt Staffing is seeking a skilled Home Health Licensed Practical Nurse (LPN) for an assignment in Bridgewater, MA. This role involves delivering compassionate, patient-focused care in a home health setting, managing a varied caseload, and maintaining accurate documentation using Homecare Homebase. Both local candidates and travelers are encouraged to apply.

Key Responsibilities:

Provide skilled nursing care tailored to individualized home health care plans.
Administer medications, perform wound care, and educate patients and families.
Monitor patient conditions and report changes to the healthcare team.
Accurately document all care provided using Homecare Homebase.
Collaborate with interdisciplinary teams to ensure comprehensive patient care.

Qualifications:

Active LPN license in Massachusetts or compact state licensure.
Minimum of 3 years of LPN experience, with at least 1 year of recent home health care experience.
Proficiency with Homecare Homebase is required.
Current BLS certification.
Reliable transportation and a valid driver’s license.
Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission.
