Position: Home Health Registered Nurse
Schedule: Full-Time

Job Overview:
We are seeking a skilled Home Health RN with proficiency in OASIS and Home Care Home Base to join our team in Medford, NY. This role focuses on delivering high-quality in-home care to patients.

Responsibilities:

Conduct patient assessments using OASIS and create individualized care plans.
Provide IV therapy, wound care, and medication administration.
Educate patients and families on chronic illness management.
Document all care using Home Care Home Base.
Collaborate with healthcare professionals to coordinate care.

Qualifications:

Active New York RN license.
Experience with OASIS and Home Care Home Base required.
Minimum 1 year of home health experience.
