Job Description

As a Home Health Registered Nurse, you will provide professional nursing care to patients in their homes, ensuring their comfort, safety, and well-being. You will be responsible for assessing patient needs, developing and implementing care plans, and collaborating with a multidisciplinary team to deliver high-quality care. Proficiency with OASIS and HCHB is required for this role.

Key Responsibilities

  • Conduct comprehensive patient assessments to identify their needs and develop individualized care plans.
  • Provide direct patient care, including wound care, medication administration, and disease management.
  • Educate patients and their families on health management and disease prevention.
  • Coordinate with physicians, physical therapists, and other healthcare professionals to ensure a cohesive approach to patient care.
  • Utilize OASIS (Outcome and Assessment Information Set) for patient assessments and documentation.

Qualifications

  • Current Registered Nurse (RN) license in the state of Arkansas.
  • Minimum of 6 months to 1 year of experience in home health care.
  • Proficiency with OASIS and HCHB required.
  • Ability to work independently and manage time effectively.
  • Valid driver’s license and reliable transportation.
