Job Description:
Sunbelt Staffing is hiring a Home Health Registered Nurse (RN) for a 13-week contract in Hampton, VA. This position is ideal for an RN skilled in OASIS documentation and experienced with Homecare Homebase software. Make a difference by providing high-quality, individualized care to patients in their homes.

Key Responsibilities:

Conduct detailed patient assessments and complete OASIS documentation accurately
Create and implement individualized care plans tailored to each patient’s needs
Deliver skilled nursing care, including wound care and medication management
Work collaboratively with healthcare teams to ensure positive patient outcomes
Provide patient and caregiver education on health maintenance and self-care

Qualifications:

Active RN license in Virginia
Proficiency in OASIS documentation and experience with Homecare Homebase required
2 years of Home Health experience preferred
Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
Locals and travelers both accepted
