Job Description:
Sunbelt Staffing is hiring an experienced Home Health Registered Nurse (RN) for a 13-week contract in Lake Oswego, OR. This role requires proficiency in OASIS documentation and experience with Homecare Homebase software, offering a fantastic opportunity to provide compassionate care in a thriving and patient-focused environment.

Key Responsibilities:

Complete patient assessments and document OASIS data
Create and manage personalized care plans based on patient needs
Provide direct nursing care, including wound care and medication administration
Work collaboratively with healthcare teams to improve patient outcomes
Educate patients and families on self-care, health management, and wellness

Qualifications:

Active RN license in Oregon
Proficiency in OASIS and experience with Homecare Homebase required
2 years of Home Health experience preferred
Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
Locals and travelers both accepted
