Home Health Registered Nurse (RN)
Location: Roanoke, VA
Contract Length: 13 weeks

Job Description:
Sunbelt Staffing is hiring an experienced Home Health RN for a contract position in Roanoke, VA. This is a great opportunity to provide quality care to patients in their homes while working with a supportive and dedicated team.

Schedule:

Monday through Friday
8-hour shifts (with potential for weekend call rotation)

Key Responsibilities:

Conduct in-home patient assessments and develop individualized care plans.
Provide wound care, medication administration, and health education.
Coordinate with interdisciplinary teams to ensure comprehensive care.
Document patient care accurately using HomeCare HomeBase (HCHB) software.
Complete OASIS documentation efficiently and in compliance with agency standards.
Monitor patient progress and make adjustments to care plans as necessary.

Qualifications:

Active Virginia RN license or compact state license.
Minimum of 1 year of recent Home Health RN experience.
Proficiency with OASIS documentation and HomeCare HomeBase (HCHB) software.
Current AHA BLS certification.
Strong organizational and communication skills.
