Sunbelt Staffing is currently seeking a dedicated and experienced Home Health LPN for a rewarding 13-week contract position in Lawrenceville, GA. The ideal candidate will have at least 1 year of home health experience and hold a current American Heart Association BLS certification.
Position Details:
- Contract Duration: 13 weeks
- Contracted Hours: 40 hours per week
- Schedule: Monday to Friday, 8:00 am - 5:00 pm
- On-call: Required at least 1 weekend per month
Requirements:
- Current LPN licensure in the state your applying for, or compact
- Minimum of 1 year of experience in home health
- American Heart Association BLS certification
Responsibilities:
- Conducting comprehensive patient assessments and reassessments to determine ongoing care needs
- Collaborating with physicians and other healthcare professionals to develop and implement individualized care plans
- Performing skilled nursing procedures, including wound care, IV administration, and medication management
- Monitoring and documenting patient progress, ensuring accurate and timely record-keeping
- Educating patients and their families on proper self-care techniques and health maintenance strategies
- Coordinating with other members of the healthcare team to ensure seamless and holistic patient care
- Participating in interdisciplinary case conferences to discuss patient care plans and make necessary adjustments
- Responding promptly to patient emergencies, providing immediate care and coordinating with emergency services as needed
- Adhering to all relevant policies, procedures, and regulations to maintain compliance with healthcare standards
- Engaging in continuous learning and professional development to stay current with evolving healthcare practices and technologies
