Job Description

Sunbelt Staffing is seeking an experienced and compassionate Licensed Practical Nurse (LPN) for a rewarding home health position. If you are passionate about making a difference in patients' lives, we want to hear from you!

Position Details:

Role: Home Health Licensed Practical Nurse
Schedule: Monday through Friday, 35-40 hours per week
Caseload: 25+ patients, focusing on a variety of needs in adult and geriatric populations

Key Responsibilities:

Deliver skilled nursing care to patients in their homes, including medication administration, wound care, and treatments.
Monitor patient conditions, documenting progress and reporting changes to the healthcare team.
Collaborate with RNs, therapists, and other team members to develop and update individualized care plans.
Provide patient and family education on health management and care procedures.

Qualifications:

Active LPN license
Minimum of 1 year of home health experience.
Ability to manage a dynamic caseload efficiently.
Valid driver’s license and reliable transportation.
