Job Description

Join a dynamic and growing home health care agency in Tulsa on a contract assignment. Local and travel candidates will be considered. We are currently seeking skilled and compassionate Registered Nurses (RNs) to fill multiple contract positions. OASIS proficiency is REQUIRED.

Key Responsibilities:

Deliver high-quality nursing care in the home setting, following established care plans.
Conduct initial patient assessments and develop customized care plans in coordination with healthcare teams.
Administer medications, provide wound care, and manage IV therapy as needed.
Educate patients and families on disease processes, treatment options, and home care best practices.
Maintain accurate and timely documentation of patient care in the electronic medical records (EMR) system.
Monitor patient progress and adjust care plans as necessary to achieve optimal outcomes.
Communicate effectively with physicians, therapists, and other care providers to ensure comprehensive care.
Adhere to all safety protocols and guidelines in delivering home health services.

Qualifications:

Current and unrestricted RN license
Previous geriatric home health experience
Strong OASIS proficiency
Experience with HCHB a big plus
Strong clinical assessment and care planning skills.
Familiarity with EMR systems and documentation standards.
Ability to work autonomously and adapt to changing patient needs.
Excellent organizational and time management abilities.
Reliable transportation and a valid driver’s license.
