We are committed to delivering high-quality care and are currently seeking an experienced Contract Home Health Registered Nurse (RN) to join us.

Contract Details:

Location: Amherst, NY
Duration: 13-Week Contract
Schedule: Full-Time, 40 Hours per Week, Monday - Friday, 8:00 AM - 5:00 PM, occasional weekends, and on-call
Compensation: Hourly pay ($55-$65), negotiable based on territory and experience
Benefits: .48c Visit to Visit Mileage Reimbursement, Medical, Dental, Vision, 401(k)
Productivity: 25-30 per week

Responsibilities:

Conduct comprehensive patient assessments in the home health setting, focusing on individualized care.
Develop and implement patient-centered care plans, emphasizing patient education and self-management.
Administer prescribed medications, conduct wound care, and provide various treatments as prescribed.
Continuously monitor patient progress, maintain accurate records, and communicate effectively with the healthcare team.
Ensure strict adherence to all relevant regulations and agency policies.
Collaborate closely with healthcare professionals, patients, and their families to optimize patient outcomes and provide compassionate support.
Provide education and support to empower patients and their families in managing their health effectively.

Requirements:

Active New York RN License.
Active AHA or American Red Cross BLS certification.
Minimum of 2 years of Registered Nurse (RN) experience.
At least 1 year of Home Health Registered Nurse (RN) experience.
Proficiency in Oasis and Homecare Homebase.
Strong interpersonal and communication skills.
Reliable transportation and a valid driver's license.
