Job Description

Sunbelt Staffing is currently seeking a dedicated and experienced Licensed Practical Nurse (LPN) for a 13-week contract position with our client, a nationwide home health agency, located in Warrick, RI.

As a Home Health Licensed Practical Nurse (LPN), you will play a crucial role in providing high-quality healthcare services to patients in the comfort of their homes. This is a full-time position with a flexible schedule that includes occasional weekends and on-call duty (typically one weekend per month).

Contract Details:

Employment Type: Full-Time, 40 hours per week, 13-week contract
Work Schedule: Monday - Friday, 8:00 AM - 5:00 PM, occasional weekends and on-call (typically one weekend per month)
Compensation: Hourly pay (negotiable based on experience and territory)
Benefits: .48c visit to visit mileage reimbursement, medical benefits, dental, vision, 401k

Job Duties:

Conduct comprehensive in-home health assessments, evaluating the patient's physical, emotional, and social needs.
Develop and implement personalized care plans in collaboration with the interdisciplinary healthcare team.
Administer medications, wound care, and treatments as prescribed by physicians.
Educate patients and their families on disease management, medications, and lifestyle modifications.
Maintain detailed electronic patient records, utilizing Homecare Homebase software.
Monitor and promptly report any changes in the patient's condition to the healthcare team.
Provide emotional support and compassionate care to patients and their families.
Ensure compliance with all relevant healthcare regulations and agency policies.

Requirements:

Active Licensed Practical Nurse (LPN) License.
At least 2 years of Licensed Practical Nurse (LPN) experience.
At least 1 year of Home Health Licensed Practical Nurse (LPN) experience required.
Proficiency in Homecare Homebase required.
Active AHA or American Red Cross BLS certification.
Must have reliable transportation.

Please Note: Due to this being a contract opportunity with limited training, one year of experience and proficiency in Homecare Homebase is required.
