Job Details:

Client: Nationwide Home Health Agency
Position: Contract Home Health LPN
Location: Pittsfield, MA
Type: Full-Time, 40 hours per week
Duration: 13-week contract with possibility of extension upon successful completion
Schedule: Monday-Friday, 8am-5pm with occasional weekends and on-call, typically once per month
Benefits: .48c mileage reimbursement, Medical, Dental, Vision, 401k eligibility after 90 days, Pay negotiable based off experience and territory, Weekly Pay

Requirements:

Active Massachusetts Licensed Practical Nurse (LPN) License
Minimum 1 year of home health experience
Proficiency in Homecare Homebase required
Active AHA or American Red Cross BLS
Reliable transportation

Job Duties:

Perform comprehensive assessments of patients' health status and develop personalized care plans tailored to their needs.
Administer medications and treatments as prescribed, ensuring adherence to established protocols and patient safety.
Monitor patients' progress closely, documenting observations and communicating any changes to the interdisciplinary team.
Provide patient and family education on disease management, medication administration, and preventive care strategies.
Utilize Homecare Homebase for accurate and timely documentation of all patient interactions and interventions.
Collaborate with healthcare professionals to facilitate continuity of care and ensure optimal patient outcomes.
