Are you a dedicated Licensed Practical Nurse (LPN) with a passion for providing high-quality home health care? Sunbelt Staffing, a nationwide healthcare staffing agency, is seeking a skilled Home Health LPN to join our team for a 13-week contract assignment with a reputable nationwide home health agency in Cambridge, MD.

Position Details:

Company: Sunbelt Staffing
Client: Nationwide Home Health Agency
Location: Cambridge, MD
Contract Duration: 13 weeks
Hours: Full-time, 40 hours per week, Monday to Friday, 8 AM - 5 PM with occasional weekends and on-call duties (typically one weekend per month).

Compensation and Benefits:

Competitive hourly pay (negotiable based on territory budget and experience).
Weekly pay.
Medical, dental, and vision benefits.
401k plan (available after 90 days).
.48c visit to visit mileage reimbursement.

Qualifications:

Active Licensed Practical Nurse (LPN) license in the state of Maryland.
Active BLS certification from the American Heart Association or American Red Cross.
Minimum of 1 year of home health experience.
Proficiency in Homecare Homebase software.
Reliable transportation and willingness to travel within the assigned territory.
Successful completion of Joint Commission onboarding requirements.

Key Responsibilities:

Provide compassionate and professional nursing care to patients in their homes.
Utilize Homecare Homebase for electronic documentation and patient management.
Conduct comprehensive patient assessments and develop individualized care plans.
Administer medications, treatments, and other prescribed interventions.
Educate patients and their families on disease management, treatment plans, and preventive health measures.
Collaborate with interdisciplinary teams to ensure coordinated patient care.
Maintain accurate and timely documentation of patient care activities.
Participate in on-call rotations and provide weekend coverage as required.
