Title: Per Diem Nurse Practitioner (NP) - Home Health Le Ray, NY & Jefferson County) - Flexible Schedule
Description:
- Are you a Nurse Practitioner seeking a flexible schedule and the opportunity to make a difference in patients' lives?
- Sunbelt Staffing is seeking a Per Diem NP to conduct home health risk assessments for Medicare Advantage plan members in Le Ray, NY and surrounding Jefferson County. (Watertown, Adams, and Fort Drum)
- This is a great opportunity for a compassionate and independent NP to work on-site with patients in the comfort of their homes.
Responsibilities:
- Conduct comprehensive annual home health risk assessments (45 minutes average).
- Evaluate patients' medical history, conditions, and medications.
- Educate patients on health conditions and medications.
- Identify gaps in care and provide recommendations.
- No creation of treatment plans or prescribing medications required.
Work Schedule:
- Flexible! Create your own schedule with shifts available Monday-Sunday.
- Minimum weekly or bi-weekly hourly commitment.
Qualifications:
- NY Nurse Practitioner License.
- Current CPR certification (BLS).
- Preferred: 6 months of current NP experience.
- Preferred: Board Certification through AANP or ANCC.
- MSN degree from an accredited school of nursing.
