Job Description

As a Home Health Registered Nurse at Sunbelt Staffing, you will deliver professional nursing care to patients in their homes, focusing on their comfort, safety, and well-being. You will conduct patient assessments, develop and implement care plans, and collaborate with a multidisciplinary team to ensure high-quality care.

Key Responsibilities

Conduct comprehensive patient assessments to identify their needs and develop individualized care plans.
Provide direct patient care, including wound care, medication administration, and disease management.
Educate patients and their families on health management and disease prevention.
Coordinate with physicians, physical therapists, and other healthcare professionals to ensure a cohesive approach to patient care.
Utilize OASIS (Outcome and Assessment Information Set) for patient assessments and documentation.
Use HCHB (Homecare Homebase) software for efficient care coordination and record-keeping.

Qualifications

Current Registered Nurse (RN) license in the state of Oregon.
Minimum of 6 months to 1 year of experience in home health care.
Proficiency with OASIS and HCHB required.
Ability to work independently and manage time effectively.
Valid driver’s license and reliable transportation.
