Job Description:
We’re hiring an experienced Home Health LPN for a full-time position in Pittsfield, MA. This role requires prior home health experience and proficiency in Home Care Home Base for documentation. Don’t wait—these positions are in high demand and fill quickly!
Responsibilities:
Provide in-home nursing care, including wound care, medication administration, and IV therapy.
Complete accurate and timely patient documentation using Home Care Home Base.
Educate patients and families on managing chronic illnesses and recovery.
Collaborate with RNs and other healthcare team members to deliver coordinated care.
Maintain compliance with all safety and care protocols.
Qualifications:
Current LPN license in Massachusetts.
Minimum 1 year of recent home health experience required.
Proficiency with Home Care Home Base.
Strong organizational and time-management skills.
