Job Description

Sunbelt Staffing is seeking a dedicated and compassionate Nurse Practitioner to join our team in Grants Pass, OR. Our client, a reputable insurance provider, requires an experienced NP to deliver exceptional patient care across Jackson and Josephine Counties. This is a fantastic opportunity to make a difference in the lives of patients by providing in-home care and ensuring a smooth transition from long-term care facilities.

Contract Details: 

  • Location: Grants Pass, OR (Covering Jackson & Josephine Counties) 
  • Employer: Sunbelt Staffing, Nationwide Healthcare Staffing Agency
  • Schedule: Monday - Friday, 8 AM - 5 PM
  • Contract Duration: 4-7 Month Contract 
  • Benefits: Medical, Dental, Vision, 401k after 90 days
  • Compensation: Hourly rate negotiated with client territory, subject to clients budget and candidate experience 

Program Details: This program offers comprehensive care for insurance members in senior living and home settings, aiming to treat patients in place when appropriate to avoid unnecessary hospitalizations. Health assessments are conducted at least monthly to provide care and resources that improve healthcare in the home setting, thus avoiding emergency room visits or hospitalizations when preventable. 

Key Responsibilities:

  • Deliver routine patient care at the patient's location with hands-on bedside care, developing care plans, and creating orders in collaboration with the primary care physician.
  • Conduct single visits to patients transitioning home as part of their discharge plan from long-term care or skilled nursing facilities, ensuring a safe and healthy transition (20-30 visits per week, ~90 minutes per visit).
  • Perform comprehensive clinical assessments for each patient.
  • Engage in advanced illness and disease trajectory discussions to establish personalized goals of care.
  • Develop a problem list and medical treatment plan for each patient.
  • Provide primary, preventive, urgent, and acute care on-site.
  • Identify and stratify high-risk patients to allow for more frequent clinical visits.
  • Develop contingency plans for high-risk conditions.
  • Communicate and collaborate effectively with the patient's primary care physician.
  • Reach out to the responsible party after each visit to establish lines of communication.

Requirements:

  • Active Nurse Practitioner license in the state of Oregon. 
  • Minimum of 2 years of experience in a clinical setting, preferably Long Term Care, SNF, or Home Health. 
  • Strong knowledge of advanced illness and disease management.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a collaborative team.
  • Experience with care coordination and managing transitions of care.
  • Valid driver's license and reliable transportation.
