Job Description

Join Sunbelt's dynamic team as a licensed Nurse Practitioner (NP) and conduct in-home assessments throughout Saline County on a per diem basis. We're seeking a highly motivated individual to cover annual In-Home Health assessments for MC Advantage Members.

Responsibilities:

Perform annual home health risk assessments for a Medicare Advantage health plan.
Evaluate prior and current medical history, medical conditions, and medication history.
Provide education on medical conditions and medications.
Identify gaps in care and offer recommendations for resolution.
No treatment plans or prescribing of medications required.

Qualifications:

Nurse Practitioner License in AR.
Current CPR certification (BLS).
Board Certification through AANP or ANCC.
Minimum 6 months of current NP experience preferred.

Education:

MSN degree from an accredited school of nursing.

Work Hours:

Flexible shifts available from Monday to Sunday.
Create your own schedule with a minimum weekly or bi-weekly hourly commitment.
