There is an open Full Time Geriatric Registered Nurse (RN) Long Term Care/Rehab job in Charlotte, NC. The Full Time Geriatric Registered Nurse (RN) will be responsible for coordinating care for residents in long term care and rehab settings. The Geriatric Registered Nurse (RN) role is full time, collaborating with a single or group of NPs, supporting their members, rounding in facilities, and coordinating member care in tandem with their care team. The Geriatric Registered Nurse (RN) will conduct regular assessments to review the resident's status and assist in achieving progress towards goals. Depending on acuity the RN will perform 5-7 assessments per day. The assessment will be conducted on Medicare Advantage plan members residing in long term care, assisted living, and senior housing facilities in Charlotte, NC and the surrounding Wake and Wayne county area. The RN is typically scheduled in one facility per shift.
